our quadratic polynomial function is:

f(x) = 4x^2 + 2x + 5

our linear binomial form is:
x - a = -2
hence:
a = x + 2

and so we have:
f(x) = 4x^2 + 2x + 5
f(a) = 4(x + 2)^2 + 2(x + 2) + 5
= 4(x^2 + 4x + 4) + 2x + 4 + 5
f(a) = 4x^2 + 18x + 25
